Escalation Logic
When a barrier stops being a one-off problem
1–2 occurrences: track the event and validate whether it repeats.
3+ occurrences: treat it as a repeat pattern, not an isolated alert.
High unit loss or repeated multi-shift impact: escalate beyond temporary recovery.
If corrective action fails repeatedly, stop recycling the same answer and recommend a durable change.
